To replace a lost, stolen or damaged card, print the form below, fill it in and post it back. If you don't want to wait for a replacement card, you can download the Service Victoria app and add your digital card to your wallet.
If your current contact details have changed and you need to update them on your Check, you can update your details before you complete this form and send it back.
If you have changed your name and need your new name on the card, please use the Change of name form.
